背景

准确的预后信息对患者，家属和临床医师至关重要。

安宁缓和医疗中对于患者30天生存期预后的评估量表PaP（Palliative Prognostic Score）, 使用Karnofsky评分（KPS）和其他五项指标来产生从0到17.5的数值分数，合计总分可划分到三个档次，估计出患者30天生存期的概率。

PaP与许多其他有效的预后量表不同的地方，在于其考虑了临床医生的主观判断。此外，PaP需要血液检验结果。

PaP的验证和使用

PaP已经在成人和儿科肿瘤学设置的大型前瞻性研究以及住院患者住院治疗，住院治疗姑息治疗单位和姑息治疗咨询团队看到的患者中进行了验证。已经证明在各种非癌症诊断中是可靠的，但是大规模验证研究尚未公开。

包括谵妄的PaP（D-PaP）

由于谵妄已被证明是一个重要的预后因素，因此加入谵妄评估指标的D-PaP也常被采用。如果临床医生使用CAM算法确定存在谵妄，患者将获得2个额外点。因此，最高分是19.5而不是17.5。在终末期癌症患者的回顾性分析中，D-PaP表现略好于PaP。

PaP的比较功效有一些比较PaP与其他预后评分系统（PPI，PPS，D-PaP）的回顾性和前瞻性研究。这些研究表明，PPI，D-PaP和PaP都能准确地识别具有不同生存概率的患者。在癌症患者中，PaP和D-PaP可能略微更准确，但需要较大的研究来验证这一发现。
